There are 3 ways in which you can contribute to Ballet San Jose:

1. You can securely donate to the Ballet using your Credit Card or on-line check.


	With this option, you can choose to donate with complete anonymity, in memory of a person or with full donor information. You can also select one-time or automatic monthly, quarterly or annual contributions. Once the transaction is completed, you will receive an email confirmation of your gift.

2. You can print out this form, and send it by US Mail with your donation
